Medication Charting Standards

The charting standards are an improvement strategy to address safety and quality issues associated with medication charting in hospitals.

The medication charting standards have been identified as the minimum requirement for safe medication charting. They are intended to be applicable wherever medicines are charted – medication charts, supplementary charts and speciality areas.

This Toolkit has been prepared for:

Those health practitioners who have been charged with piloting the National Medicines Chart and the medication charting Standards and/or conducting education and training on how to use the National Medicines Chart and medication charting Standards as a component of a quality assured safe medication management process within a New Zealand hospital.

Toolkit Introduction

The Toolkit has two major section:

Tools - a training aid to ensure a nationally consistent understanding about standardising documentation and recording of medicine therapy for patients and consumers. It includes form templates which can be copied and used by staff during the medication charting pilot. The Tools section also includes additional information to enhance understanding of the importance of a National Medicines Chart and Medication Charting Standards to ensure safe and effective medicine management practices, thereby assisting in the main objective of reducing adverse drug events

Tools

Trainer’s guide - a set of masters upon which training is based that can be copied and provided to the attendees of the National Medicines Chart and medication charting Standards education and training sessions. It consists of a list of important terms; how to fill in the National Medication Chart; how to apply the medication charting Standards and how and what should be measured and reported.

Workbook

The Workbook is a reference guide for health practitioners to complete the medication chart to ensure medication charting is undertaken in accordance with the standards for charting medication as well as to understand the design elements of the national medicines chart.
