Primary Health Care

The Safe Medication Management Programme encompasses the entire health and disability sector.  The SMM Programme is focusing on the improving the medicine information interface between DHBs and primary care as a priority. The programme has begun working with representatives from across the primary sector on various initiatives involving obtaining accurate medicine lists at admission and discharge.  It is essential that hospitals have a complete understanding of what a patients is taking in terms of medicines before they were admitted to hospital and that patients and primary care practitioners have a complete undertsanding of any changes to medicines that occured in hospital on discharge back home.
