Primary Health Care

The Safe Medication Management Programme encompasses the entire health and disability sector. Initially, the SMM Programme is focusing on the interface between DHBs and primary care as a priority.The programme has begun working with representatives from across the primary sector on the two key areas of admitting and discharging people from hospital. It is essential that hospitals have a complete understanding of a person’s medication history when they are admitted to hospital and that primary care practitioners have a complete history on discharge.
